If you received your COVID-19 shot(s) through the VA, you will not automatically be entered into the Oregon TAKE YOUR SHOT drawing and giveaway. There is a form that you will need to fill out in order to be entered into the drawing. The deadline to submit the form is 6/27/2021. You can complete and submit the form at: https://appengine.egov.com/apps/or/take-your-shot.
